So, on one level, the aftereffects of trauma, conditioned obedience, and identity destruction have left Bucky very vulnerable to the manipulation of his emotions and beliefs. The doctor tells Bucky things that sound plausible, even comforting, but are actually incredibly toxic lies. He undermines Bucky's trust in himself and isolates him from his support network. He reinforces Bucky's worst fears about himself. And then, as Bucky gets moodier, more withdrawn, and more temperamental, that just puts him more in the doctor's power: he should see more often... I'm afraid we need to have him institutionalized, for his own good, just for a few weeks...
And, on another level, the therapist is salaciously fascinated by Bucky's abuse by Hydra. He gets Bucky to recount his rapes and humiliations, in increasingly lurid detail as Bucky falls more under his influence. Maybe this culminates with the therapist himself raping Bucky.
Don't care whether this guy is just a terrible person or whether this is a scheme by Hydra.
